Au Moyen Âge, Mühlhausen était une ville-État. Le 8 octobre 1356 l'église avancée a été placée dans un document de l'archevêque de Mayence Gerlier de Nassau en service et était depuis lors jusqu'à la Réforme, sous les chevaliers teutoniques.
En 1430, la ville entra en relation avec Erfurt et Nordhausen et forma avec celles-ci la Ligue des trois Villes.
