Il territorio della diocesi è interamente distribuito sulla provincia di Alessandria e comprende i comuni di Alessandria, Borgoratto Alessandrino, Carentino, Casal Cermelli, Predosa, Castelspina, Bassignana, Rivarone, Valenza, Alluvioni Cambiò, Isola Sant'Antonio, Castellazzo Bormida, Felizzano, Quargnento, Frugarolo, Oviglio, Pietra Marazzi, Gamalero, Pecetto di Valenza, Montecastello, Bosco Marengo, Pasturana, Piovera, Tassarolo, Frascaro, Solero e Capriata d'Orba.
