La catedral va ser construïda al segle XV (iniciada en 1424) per monjos dominics de Guimarães.
Després de la creació de la diòcesi de Vila Real el 1922 pel Papa Pius XI, l'església va ser elevada a la categoria de catedral el 1924 .
És un exemple notable de l'arquitectura gòtica tardana al nord de Portugal. La façana principal es caracteritza per un portal de tres arcs apuntats, envoltat de robustos contraforts i coronat per una rosassa . Al segle XVIII es van afegir el campanar i la capella actuals, amb elements barrocs com una torre quadrada amb balustrada i cúpula.
L' interior és relativament sobri , amb sostres de fusta i una decoració limitada. Hi ha alguns nínxols funeraris medievals i un altar major manierista portat del Monestir d'Odivelas en 1938. En 2003, es van afegir vidrieres modernes dissenyades per l'artista João Rodrigues Vieira .
