Santa Maria Assunta i Fornovo er en romansk stil, romersk-katolsk sognekirke eller pieve beliggende i Fornovo di Taro i regionen Emilia Romagna, Italien.
En kirke på stedet er dokumenteret siden 854, og blev oprindeligt genopbygget i det 11. århundrede. Klokketårnet blev tilføjet i 1302. De første to kapeller blev først tilføjet til kirken i det 14. og 16. århundrede. Yderligere kapeller blev tilføjet under rekonstruktioner fra 1712-1745. En rekonstruktion i 1927-1945 ændrede kirken og facaden tilbage til dens romanske elementer.
