Santa Maria Assunta a Fornovo is een Romaanse, rooms-katholieke parochiekerk of pieve gelegen in Fornovo di Taro in de regio Emilia Romagna, Italië.
Een kerk op de locatie is gedocumenteerd sinds 854 en werd aanvankelijk herbouwd in de 11de eeuw. De toren werd toegevoegd in 1302. De eerste twee kapellen werden pas in de 14de en 16de eeuw aan de kerk toegevoegd. Verdere kapellen werden toegevoegd tijdens de reconstructies van 1712-1745. Een reconstructie in 1927-1945 herstelde de kerk en de gevel naar zijn Romaanse elementen.
