Der Schleemer Bach entspringt in Hamburg-Rahlstedt im kleinen Waldgebiet Hegen in der Nähe von Rahlstedt-Ost. Er fließt am Kinderkrankenhaus Wilhelmstift vorbei und ist parallel zur Grunewaldstraße auch die Landesgrenze. Dann fließt er meist in südlicher Richtung am Hauptfriedhof Öjendorf und Öjendorfer See vorbei. Für diesen ist er Zu- und Abfluss zugleich. Der Jenfelder Bach und die Barsbek münden in ihn. An der Grenze von Hamburg-Billstedt und -Billbrook nahe der Roten Brücke mündet er nach etwa 10 Kilometern in die Bille.
In der Nachkriegszeit verkam das Flüsschen teilweise zu einem Abwassergraben. Dies wurde aber später zurückgebaut. Im Bereich Wilhelmstift wurde ein Feuchtgebiet zurückgebaut, das über eine Holzbrücke überquert werden kann.
