Did you know that 6 men were sent to assassinate the Archduke Franz Ferdinand of Austria in 1914. One of them was supposed to throw a bomb but missed the target. He immediately took a cyanide pill to kill himself and jumped into the river. The cyanide was not active enough and only caused vomiting. And the river was also less than half a meter deep from the hot summer. So the terrorist failed three times, once to kill Franz Ferdinand, and twice to kill himself.
