The name Bilstain has an Old Dutch origin. In 1125, it was first mentioned in writing as Bilisten, where sten stands for stone and bili for grooved. This possibly referred to a rock formation in the current Bois Saint-Jacques near the village, now called Nez de Napoléon.
The forest and part of the territory of the village were donated to the Saint-Jacob's Abbey in Liège in 1125. Another part belonged to secular lords, who had their seat in the donjon north of the village.
