The leprosarium was established for the care of leprosy patients and is first documented towards the end of the 13th century. From 1423, the foundation was managed by Lübeck. In 1479/80, the house was rebuilt from the estate of Lübeck Mayor Andreas Geverdes. It is a single-story brick building, preserved in its main features to this day, measuring 26.5 m in length and 11.4 m in depth, which is divided into two halves by a central cross wall, with the southern half designated for men and the northern half for women. The simultaneously enacted house rules provided for the accommodation of six men and six women in each half of the house. Later, the house was used as a poorhouse, again for six men and six women each. In 1787, the interior of the house was renovated; in 1844 it received a new roof. Three inscription plaques commemorate the start and completion of the construction in 1479/80, as well as the renovation of 1787. The leprosarium generated income from the rental proceeds of the Klein Grönau estate belonging to the leprosarium and from the interest on invested capital. For a long time, the administration was associated with the Westerau Foundation. The supervisors of the foundation were the two oldest Lübeck mayors as well as the two elders of the guild of tailors. In 1843, 37 people were registered on the 3-hectare estate, which is almost entirely surrounded by the Lauenburg village of Groß Grönau.
