El nombre Bilstain tiene un origen en el Antiguo Neerlandés. En 1125 fue mencionado por primera vez por escrito como Bilisten, donde sten significa piedra y bili significa surcado. Posiblemente se trataba de un afloramiento rocoso en el actual Bois Saint-Jacques cerca del pueblo, hoy conocido como Nez de Napoléon.
El bosque y una parte del territorio del pueblo fueron donados en 1125 a la abadía de San Jacobo en Lieja. Otra parte pertenecía a señores seculares, que tenían su sede en la torre del homenaje al norte del pueblo.
