Deesem est situé dans le sud-est de la ville de Lohmar. Les localités environnantes sont Wahlen et Effert au nord, Krahwinkel à l'est, Pohlhausen au sud-est, Winkel et Breidtersteegsmühle au sud, Breidt au sud-ouest et Grimberg à l'ouest.
Deesem est habité depuis très longtemps et est souvent mentionné dans les livres d'histoire avec Breidt, Inger et Birk, et est étroitement lié à la paroisse de Birk. Le chemin de l'église de Birk, également appelé Lichweg ou Leichweg, est souvent décrit en relation avec le village de Deesem.
