L'incidente del volo Alitalia 404 fu un incidente aereo avvenuto alle ore 19:11 del 14 novembre 1990 nei pressi di Weiach, comune del Canton Zurigo in Svizzera, causato, durante la fase di discesa verso lo scalo aeroportuale di destinazione, l'Aeroporto di Zurigo, da un malfunzionamento del sistema Instrument landing system e dalla conseguente errata valutazione dell'altitudine da parte del comandante.
Nell'incidente perirono tutti i quaranta passeggeri e i sei membri dell'equipaggio: Il Comandante Raffaele Liberti, il primo ufficiale Massimo De Fraia e gli assistenti di volo Adalberta Pollidori, Vittorio Caronti, Francesca Pigozzi e Marco Gismondi.
