Il paese si stende in longitudine lungo le pendici meridionali dell'omonimo monte, a 748 m s.l.m., ed è una stazione climatica frequentata in estate per il clima mite e asciutto.
Il territorio comunale include due exclave: Palazzaccio, compresa tra i comuni di Borgo Pace e Sestino, e Torre Fossati, compresa tra i comuni di Belforte all'Isauro , Mercatello sul Metauro , Sant'Angelo in Vado e Sestino .
Il territorio del comune è interessato dal Parco naturale regionale del Sasso Simone e Simoncello di cui è la sede capoluogo.
