Agios Efstratios werd sinds de jaren dertig gebruikt om politieke gevangenen in ballingschap gevanen te zetten. De eerste communisten werden verbannen naar het eiland met de Idionymon-wet van Venizelos, later tijdens het Metaxas-regime en opnieuw tijdens de Griekse burgeroorlog. Na Makronisos werd het het grootste gevangeniseiland in de Egeïsche Zee. Tijdens de Griekse militaire junta van 1967-74 werd het eiland opnieuw gebruikt als gevangenis voor politieke dissidenten. Tot de ballingen hier behoorden Ilias Iliou, Stefanos Sarafis, Dimitris Glinos, Kostas Varnalis, Tassos Leivaditis, Yiannis Ritsos, Menelaos Lountemis, Manos Katrakis en Mikis Theodorakis.
