Jernbanestrækningen Mailand–Genua er en italiensk jernbanestrækning fra Rete Ferroviaria Italiana. Den er 157 kilometer lang og en hovedstrækning. Persontransporten her drives af Trenitalia.
I modsætning til jernbanestrækningen Turin–Genua blev strækningen Mailand–Genua til ved en sammenlægning af eksisterende strækninger. En af disse sektioner var strækningen Rogoredo–Pavia, som afveg fra strækningen Mailand–Piacenza og blev taget i brug den 10. maj 1862. Før dette, den 25. januar 1858, blev strækningerne Alessandria–Tortona–Voghera–Casteggio og Tortona–Novi Ligure åbnet. Disse skabte en forbindelse til den allerede eksisterende jernbanestrækning Turin–Genua. Den 14. november 1867 blev forbindelsen mellem Genua og Mailand fuldendt med sektionen Pavia–Voghera.
