Opperzau ligt noordoostelijk in de gemeente Windeck, direct aan de Sieg en nabij een bosgebied. Samen met het rheinland-pfälzische Oppertsau vormt Opperzau een aaneengeschakeld bebouwingsgebied.
Een 200 meter lang stuk van de rijkswegen 112 / 267, dat Opperzau en Oppertsau scheidt, vormt niet alleen de gemeentegrens, maar is ook de staatsgrens tussen Noordrijn-Westfalen en Rijnland-Palts. Bovendien heeft de weg aan elke kant een eigen naam: Siegstraße voor de Oppertsauer en Fürthener Straße voor de Opperzauer kant.
