La Ernstbach prend sa source à 500 m au nord-est de la Hof Mappen dans la forêt de l'arrière-pays rhénan, sur le territoire de la ville d'Eltville am Rhein, près de l'ancien Gebück rhénan. Quelques mètres en dessous de la source, il croise la ligne de Gebück et traverse, sur son chemin vers l'ouest, le domaine de la Hof Mappen, qui ne fait plus partie du Rheingau. Ensuite, il entre dans la forêt de l'arrière-pays, qu'il ne quittera plus jusqu'à son embouchure. Ce faisant, il décrit, à l'écart de sa direction générale vers l'ouest, un large arc vers le nord autour du Ziemerskopf. Ensuite, il reçoit en tant qu'affluent principal, à gauche, le Wickersheller Bach au niveau du pont de Wickersheller.
