Das Klinikum Mittelbaden Rastatt-Forbach, Klinik Forbach, vormals Kreiskrankenhaus Forbach, ist ein Krankenhaus der medizinischen Grundversorgung und verfügt über 60 Betten. Es liegt in Forbach im Nordschwarzwald und gehört zur Klinikum Mittelbaden gGmbH.
Vorläufer des heutigen Klinikum Mittelbaden Forbach ist das im Jahr 1896 gegründete Bezirksspital Forbach. Die Gemeinden Forbach, Gausbach, Bermersbach, Langenbrand sowie die Murgschifferschaft bildeten damals den sogenannten Bezirksspitalverband, die das Forbacher Krankenhaus gründeten und das mit 15 Betten zur stationären Versorgung in Betrieb ging.
