Santi Jacopo e Cristoforo ist eine kleine römisch-katholische Kirche oder Kapelle im Dorf Bolgheri, in der Gemeinde Castagneto Carducci, Provinz Livorno, Toskana, Italien.
Eine Kirche dieses Namens befand sich um das Jahr 1000 im Stadtburg. Der Antipope Nikolaus V fand hier 1329 Zuflucht. Nachdem die Burg 1492 zerstört wurde, wurde die heutige Kirche zwischen 1505 und 1515 aus Steinen dieser Burg errichtet. Eine Wand stammt aus mittelalterlicher Bauweise. Die Kirche ist San Jacopo Baronto von Pistoia gewidmet. Die Steinkirche wurde 1902 unter der Schirmherrschaft von Graf Alberto Guido umfassend restauriert. Das Portal hat ein Marmorschild der Familie Della Gherardesca. Das Innere ist mit Kunstmarmor dekoriert, während der Eingang zur Sakristei Darstellungen der heiligen Patrone zeigt.
