Het werd in 1115 als vesting gebouwd door Gerhard I en was de voorouderlijke zetel van de familie Von Blankenheim.
De heren van Blankenheim kregen in 1380 de titel van graaf.
De graven van Blankenheim fuseerden met de graven van Manderscheid en werden vanaf 1469 Manderscheid-Blankenheim genoemd. Met de uitsterving van de lijnen Manderscheid-Gerolstein en Manderscheid-Kail werd het graafschap Manderscheid het grootste autonome territoriale gebied van de Eifel.
