Der Name Bilstain hat einen alt-niederländischen Ursprung. 1125 wurde er erstmals schriftlich als Bilisten erwähnt, wobei sten für Stein steht und bili für geriffelt. Möglicherweise handelte es sich um ein Felsgebiet im heutigen Bois Saint-Jacques in der Nähe des Dorfes, das heute Nez de Napoléon genannt wird.
Der Wald und ein Teil des Gemeindegebiets wurden 1125 an die Sint-Jacobsabtei in Lüttich geschenkt. Ein anderer Teil gehörte weltlichen Herren, die ihren Sitz im Donjon nördlich des Dorfes hatten.
