Opperzau est situé au nord-est de la municipalité de Windeck, directement sur la Sieg et près d'une zone forestière. Avec l'Oppertsau en Rhénanie-Palatinat, Opperzau forme une zone d'habitation contiguë.
Un tronçon de 200 mètres de la route nationale 112 / 267, qui sépare Opperzau et Oppertsau, représente non seulement la frontière communale, mais est également la frontière entre la Rhénanie du Nord-Westphalie et la Rhénanie-Palatinat. De plus, la route porte un nom propre de chaque côté : Siegstraße pour les Oppertsauer et Fürthener Straße pour le côté Opperzauer.
