De bouw begon op 17 april 1565 om een kerk te bouwen voor de Orde van Ridderschap van Sint Stephanus, opgericht door de Groothertog Cosimo de' Medici om Saraceense piraterij in de Middellandse Zee te bestrijden. Het project, met ontwerpen en toezicht van Giorgio Vasari en David Fortini, had als doel een kerk te bouwen op de locatie van een oudere kerk genaamd San Sebastiano alle Fabbriche Maggiori, die dateert van ten minste 1074. De nieuwe kerk op de locatie werd geheiligd op 21 december 1569. De façade, in wit marmer, werd ontworpen door Don Giovanni de' Medici, de onwettige zoon van Cosimo I, met de hulp van Alessandro Pieroni; hun ontwerpen werden gekozen boven het oorspronkelijke plan van Vasari. Een inscriptie herdenkt de voltooiing tijdens het bewind van Ferdinando I de' Medici.
