Klinikum Mittelbaden Rastatt-Forbach, Klinik Forbach, tidligere Kreiskrankenhaus Forbach, er et hospital for medicinsk grundlæggende behandling og har 60 sengepladser. Det ligger i Forbach i Nordschwarzwald og tilhører Klinikum Mittelbaden gGmbH.
Forgængeren for det nuværende Klinikum Mittelbaden Forbach er det i 1896 grundlagte distriktshospital Forbach. Kommunerne Forbach, Gausbach, Bermersbach, Langenbrand samt Murgskipperforeningen dannede dengang den såkaldte distriktshospitalforening, der grundlagde hospitalet i Forbach, som blev taget i brug med 15 senge til stationær behandling.
