L'église Santa Maria Assunta a Fornovo est une église paroissiale catholique romaine de style roman située à Fornovo di Taro dans la région d'Émilie-Romagne, en Italie.
Une église à cet endroit est documentée depuis 854 et a été initialement reconstruite au 11ème siècle. Le clocher a été ajouté en 1302. Les deux premières chapelles n'ont été ajoutées à l'église qu'aux 14ème et 16ème siècles. D'autres ont été ajoutées lors des reconstructions de 1712 à 1745. Une reconstruction de 1927 à 1945 a rétabli l'église et la façade à ses éléments romans.
